'10/1/02 Gentleman called and wanted a complaint form sent to him. He
wanted to explain to me over the phone nature of complaint. I told
him I'd send him complaint form and brochure and if he has any
questions about procedure to call us.
'10/16/02 Gentleman called and requested two complaint forms to be mailed
to him.
10/29/02 Gentleman came into office and picked up complaint form.
